Florida Hospital Wesley Chapel has provided faith-based, whole person care to the community since it opened in 2012. Every detail of our 83-bed facility is purposefully designed to create an environment of healing for mind, body and spirit. Our hospital is equipped with state-of-the-art equipment from outpatient services to our surgical suites and inpatient care. In 2013, we also opened a 50,000-square foot Health and Wellness Center that includes an indoor track, group exercise areas, and an aquatics center with a lap pool to help community members achieve optimal health. In August 2015, the hospital began construction on am expansion that will open in December of 2016, significantly increasing the number of inpatient beds, surgical suites, and emergency department rooms.

 

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:

·         Acts as a liaison between the hospital and patients, physician office staff, patients’ families to determine the customers’ needs and satisfaction with services provided

·         Demonstrates collaboration and teamwork in managing patient flow

·         Provides education and support to cancer patients by connecting them to resources that are available

·         Develops and maintains patient informational literature and resource library

·         Triages telephone calls, provides requested information as needed

·         Assists in scheduling bundled appointments for services collocated in the women’s center

·         Participates in the Cancer Support Groups and attends Tumor Board Meetings

 

 

 

Qualifications

KNOWLEDGE AND SKILLS REQUIRED:

·         Demonstrates ability to communicate professionally

·         Demonstrates strong organizational skills

·         Proficient in Microsoft office products

·         Skilled in providing education and program development.

·         Skilled in communications with multidisciplinary teams, patients, families and surgical teams

·         Community education and public speaking

 

 

EDUCATION AND EXPERIENCE REQUIRED:

·         Bachelor’s degree in Nursing

·         Five or more years of experience as a nurse navigator

·         Experience in women’s health, breast health and/or related women’s services

 

 

EDUCATION AND EXPERIENCE PREFERRED:

·         Experience in women’s diagnostic imaging and outpatient imaging

·         Experience in program development

 

 

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

·         Current registration with Florida State Board of Nursing as a registered professional nurse

·         Basic Life Support (BLS)
